Admissions Checklist

KINDERGARTEN

Pre-Kindergarten and Kindergarten Students

 

All students:

• Completed Application for Admission Form

• Copies of previous and current school reports

• 2 current passport sized photograph of student

 

Indonesian Students / Students from Indonesian Schools:

• Original Birth Certificate (for sighting) and 1 copy for files

• Original Family Registration Card (for sighting) and 1 copy for files

 

International Students:

• Original Passport and KITAS of parent (for sighting) and 1 copy for files

 

ELEMENTARY SCHOOL

Grade 1 - 6 Students

 

All students:

• Completed Application for Admission Form

• Copies of previous and current school reports

• Copies of any standardized test results

• 2 current passport sized photograph of student

 

Indonesian Students / Students from Indonesian Schools:

• Original Birth Certificate (for sighting) and 1 copy for files

• Original Family Registration Card (for sighting) and 1 copy for files

• Reference Letter from former school 

 

International Students:

• Original Passport and KITAS of one Parent (for sighting) and 1 copy for files

• Original Passport of student and 1 copy for files

• Official School Transcript 

 

MIDDLE SCHOOL

Grade 7 - 9 Students

 

All students:

• Completed Application for Admission Form

• Copies of previous and current school reports

• Copies of any standardized test results

• SWA Student Statement Form

• 2 current passport sized photograph of student

 

Indonesian Students / Students from Indonesian Schools:

• Original Birth Certificate (for sighting) and 1 copy for files

• Original Family Registration Card (for sighting) and 1 copy for files

• Copy of STTB and UNAS results

• Reference Letter from former school

 

International Students:

• Original Passport and KITAS of one Parent (for sighting) and 1 copy for files

• Original Passport of student and 1 copy for files

 

HIGH SCHOOL

Grade 10 - 12 Students

 

All students:

• Completed Application for Admission Form

• Copies of previous and current school reports

• Copies of any standardized test results

• SWA Student Statement Form

• 2 current passport sized photograph of student

 

Indonesian Students / Students from Indonesian Schools:

• Original Birth Certificate (for sighting) and 1 copy for files
